Job Summary and Qualifications What You Will Do In Your Role: You will review patient case lists generated by technology-enabled data triggers to identify patients that could benefit from intervention, while tracking outcomes of referrals and treatments and maintaining detailed records that measure effectiveness of the program. You will review clinical progress of patients and coordinate communication to referring, consulting, and primary care physicians, notifying an MD if there is a clinically abnormal finding. You will evaluate and provide input regarding the utilization of resources to meet the needs of the target population. You will plan and deliver weekly status updates. You will maintain and role models a leadership/customer service role in interacting with colleagues, physicians, guests, patients, and vendors.What Qualifications You Will Need: Graduated from an accredited school of nursing with an Associates or Bachelor of Science in Nursing (BSN) is preferred. Current Registered Nurse License in the State of Texas or Multi-State Compact License (Employees with RN Compact License are required to obtain Texas RN License within 90 days of hire date). Bilingual and able to read, write and speak Spanish proficiently is preferred Knowledge and application of pertinent computer programs and imaging software (I.E. MUSE, PACS, etc.) Masters degree preferred.This role requires you to be fully vaccinated for COVID-19 based on local, state and /or federal law or regulations (unless a medical or religious exemption is approved). Our Emergency Room is a Level III Trauma Center and we have extended our ER services into the Rio Grande Valley with our three remote ERs in McAllen/Mission, Edinburg, and San Juan. Our Women's Services include L&D and a 30+ bed NICU. We also have an entire children's hospital within our main facility to care for the children in our community. We work with the Texas Transplant Institute, which serves local patients in need of organ, tissue or stem cell transplants.
