The RN Nurse Navigator acts as a primary point of contact for
patients after initial diagnosis, and until patients have
sufficient knowledge and resources to self-direct care. Has
responsibility for monitoring, coordinating care, and educating
cancer patients and families as they move through the process of
healing. The RN Nurse Navigator will contact patients by phone and
in person to establish plans of care. It is the goal of the RN
Nurse Navigator to assess the care and services required by cancer
patients and make appropriate referrals and empower them through
knowledge and emotional support to independently navigate cancer
services. The RN Nurse Navigator will also co-lead the Tumor Board
Meetings and facilitate the meeting communications with all staff
members. The RN Nurse Navigator is instrumental in clinical
improvements with patient care processes and improving the clinical
experience for the patients and physicians.
* Three years previous oncology nursing experience preferred.
* Demonstrated skill in coordinating care needs required.
* Candidates with previous experience in program development will
* Bilingual preferred (Spanish)
* Must demonstrate commitment and adherence to STHS's Compliance
Program and Code of Conduct through compliance with all policies
and procedures, the Code of Conduct, attendance at required
training and immediately reporting suspected compliance issue(s) to
the Compliance Officer.
EDUCATION / LICENSURE:
* Must obtain Breast Patient Navigator Certification (BPNC) or
Certified Breast Cancer Nurse (CBCN) within 24 months of start
date. If current with certification must maintain certification
while in position.
* Registered Nurse currently licensed in the State of Texas, or
part of a compact state license, or has a valid temporary RN
license for the State of Texas while waiting to receive the
permanent license. BSN preferred.
* Effective January 6, 2016, must possess an Associate Degree,
Baccalaureate Degree or Diploma from a School of Nursing or
* Must have and maintain current certifications, according to unit
specific needs. Certification must be in accordance with the
curriculum of the AHA, for Healthcare Provider's Program (for any
and all certifications applicable through the AHA program)
